Job Summary:
Do you enjoy a job in which no two days are the same? We are seeking an LMSW-cc to join our hospice & homecare team in the Augusta area providing care to patients in the local community.

Position Highlights:
Provide care in varied settings including home, long term care, or assisted living.
Work closely with an incredible interdisciplinary team developing care plans to meet the complex needs of patients and families as they approach end of life.

The ideal candidate will have:

Licensed at the Masters level (LMSW-cc) by the State Board of Social Work licensing required.
Maine Driver’s License required
1 year or more of pertinent experience preferred

Job Description:

Implements assigned patient treatment plan and completes all required documentation.

Systematically and continually assesses the patient’s mental status in collaboration with the patient/family/providers.

Provides outpatient group, individual and family counseling.

Identifies and refers patients to appropriate community resources for follow-up care.
